Responsibilities:
o Perform supervisory/quality assurance visits to check on status of cases.
Review log book/notes.
Observe caregiver(s) to assess appropriateness and rapport.
Assess client/family/facility satisfaction with client services and determine if any changes to care plan and/or caregiver are needed.
Goal is a minimum of 7 quality assurance client visits per week.
New clients are visited within the first 7 days and existing clients are visited at least every 30 days.
Keep tracking system for all quality assurance visits needed and completed.
(KPI) o Regularly evaluate and discuss need for increase in service/billable hours as appropriate.
(KPI) o Use Home Care Pulse survey monthly to review feedback from clients and caregivers, and use the tool to improve client client and caregiver satisfaction.
(KPI) o Actively pursue new and existing clients and caregivers to bolster Nest & Care's digital reputation by asking for reviews on Google, Facebook and/or Caring.
com o Create documentation of all quality assurance visits in system.
(KPI) o Evaluate clients to determine appropriate service mix, including activities, personal care needs and need for outside services.
Evaluate and document safety issues and concerns and plan for addressing.
o Responsible for caregiver performance evaluations.
(KPI) o Troubleshoot caregiver performance concerns.
Remove caregivers from cases as appropriate if serious infractions or complaints occur.
Coordinate all actions with Staffing Department, Operations Coordinator, Administrator/Designee.
o Subsequent to disciplinary action and/or performance concerns, perform follow up supervision of caregivers to assess adherence with performance expectations.
(KPI) 1 o Document caregiver compliments, concerns and complaints in system.
o Responsible for caregiver retention activities in collaboration with Administrator/Operations Coordinator.
o Provide direct client care as needed.
o On call rotation (every other week) o Complete client intakes, assessments and care plans as needed, and input into software program.
(KPI) o Work with Staffing Coordinator to ensure effective staffing of cases at all times.
o Open new cases and complete service agreements and related forms with clients/families as needed/directed.
o Any assessments performed must be coordinated with the Staffing Department and Director of Nursing.
o Discuss home care services with potential clients and referral sources both over the phone and in person (home assessment).
o Walk caregiver(s) into case (i.
e.
home, facility) on first visit to introduce caregiver(s) to client/family/facility and ensure caregiver(s') understanding of care plan.
o Train caregivers (fill-in/permanent/temporary) per care plan.
Responsible in making sure caregiver is trained and equipped to implement the care plan.
o Liaise with existing referral sources in an effort to foster and maintain excellent working relationships and generate ongoing referrals/business.
Perform marketing functions as directed.
o Perform other duties as assigned.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
